Work hours/days: Mon- Fri 8am to 5pm
This job will have the following responsibilities:
- Scheduling patients from hospital to home.
- Follow up appointments for the QTC patients.
- Medical Knowledge important
- Able to work with excel; entry
- Working with a team to schedule QTC appointments (Hospital to Home) for contracted and affiliated patients to aid in limiting readmissions.
Good grammar and writing skills
possible medical knowledge
Electronic Medical Record knowledge
Excel knowledge
