As a Clinical Care Coordinator, you will...
- Facilitate clinical care coordination services for clients in need of enhanced care coordination.
- Ensure that clients receive timely and efficient support services though a multi-disciplinary care plan including referrals to both internal and external resources.
- Perform data collection, reporting, and surveying.
- Oversee the coordination of care across programs.
- Participate in Continuous Quality Improvement activities.
- Participate in huddles.
- Schedule and assure patient access to care.

To qualify for the position, you will need...
- An associates degree
- At least one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity.
- Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
