Nurse Practitioner (NP) - In-Home Wellness Assessments
Location: Canyon County Idaho

We're a leading healthcare organization dedicated to improving patient outcomes. We're seeking a compassionate and experienced Nurse Practitioner to conduct in-home wellness assessments for Medicare beneficiaries.
Your Role:
As a Nurse Practitioner, you'll play a vital role in enhancing the health and well-being of our members. Your responsibilities will include:
- In-Home Assessments: Conducting comprehensive health assessments in patients' homes.
- Medical Reviews: Reviewing medical history, medications, and functional status.
- Patient Education: Providing education on health management, medication adherence, and disease prevention.
- Care Gap Identification: Identifying and addressing care gaps to improve patient outcomes.
- Collaboration: Collaborating with healthcare teams to ensure seamless care coordination.
- Licensure: Valid Nurse Practitioner license in the state of [State]
- Experience: Previous experience in primary care, geriatrics, or home health is preferred.
- Clinical Skills: Strong clinical assessment and diagnostic skills.
- Interpersonal Skills: Excellent communication and interpersonal skills to build rapport with patients and healthcare providers.
- Technological Proficiency: Proficiency in electronic health records (EMR) and telehealth technologies.
