STAFF NURSE- RENAL/ PANCREAS TRANSPLANT- COORDINATOR/ Full time 40 Hours/ Variable/ Day Shift/ BWH Boston / Ambulatory nursing

  • The outpatient Transplant Nurse Coordinator is a professional nurse who is responsible for overseeing the transplant process. Transplant patients are an extremely complex patient population and, as such, the Transplant Coordinator must possess a very strong clinical background.
  • This nurse must be very well organized, have good attention to detail, demonstrate the ability to coordinate and follow through on all aspects of the patient's care, and effectively communicate with all members of the transplant team, the patient and the family.
  • The RN should be registered in Massachusetts and responsible and accountable for a specified group of patients.
  • Demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs.
  • Demonstrates the ability to effectively manage patients by assuming full responsibility for assessment, planning, implementation and evaluation of patient care.
  • The Coordinator is directly responsible to the Program/Surgical Director, Medical Director, Nurse Manager and Nursing Director of the Renal/Pancreas Transplant Program.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Participates in multidisciplinary team meetings.
  • Attends scheduled Transplant clinics, as well as ad hoc clinics when clinically needed.
  • Communicates patient information to the team via appropriate documentation and team conferences. This information will include assessment of response to treatment after discharge and long term effects.
  • Initiates patient contact post- discharge to monitor patient progress. Conducts post-acute patient follow-up to insure execution of follow-up care, non-acute plan of care, optimal service utilization; and the quality of care and service to patients post-transplant.
  • Manages and continually improves systems to provide information on patient's current level of care/changes in clinical status to the team, third party payers, referring physicians and others.
  • Utilizes the electronic medical record and other information technologies in the care of patients.
  • Works with the Administrative Coordinator/Practice Assistant to make sure all out-patient testing is scheduled and being followed up.
  • Arranges and follows up on all testing being done as an out-patient, under the guidance of the transplant surgeons and nephrologists.
  • Communicates with referring MDs' offices to keep them up to date on mutual patients. Assists the referring office in getting any necessary test results.
  • Assumes responsibility for initiation of clinical assessment, initial identification of problems post-transplant.
  • Reviews, revises and initiates development of patient teaching materials with team members.
  • Assists with any insurance audits that require compiling and researching patient information.
  • Reviews all testing results after transplant clinic sessions and adjusts per protocol.
  • Manages the care of patients throughout the post-transplant course.
  • Covers for nurse colleagues as needed for patient care.
  • At the discretion of the Nurse Manager, Nurse Director, and/or Medical and Surgical Directors respectively, nurses are cross trained to assist in the comprehensive coverage of all kidney/pancreas transplant nursing roles including but not limited to pre-, living donor, and post-transplant.

    • Recommends changes in policies and procedures through appropriate channels.
    • Participates in determining and implementing goals and objectives for the transplant program.
    • Participates in routine group discussions to assist in process improvement.
    • Please note there will be no on-call responsibilities.


    Qualifications
    QUALIFICATIONS:
  • Graduate of an approved school of nursing with current registration in Massachusetts. BSN required for newly licensed nurses.
  • Prior experience (2 years or more) in abdominal surgery or transplant-focused patient care required.
  • Prior experience in transplant-focused patient care strongly preferred.
  • Prior experience (1 year or more) as a clinical transplant coordinator strongly preferred.
  • Prior experience in ambulatory nursing strongly preferred.
  • Telephone-based clinical assessment technique preferred.
  • ABTC Certified Clinical Transplant Coordinator preferred.

  • Skills, Competencies, and Abilities:

  • Problem Solving: Ability to address problems that are highly varied and complex, often requiring staff input, innovation, creativity, often understanding and utilizing diagnostic results to resolve issues or care plan.
  • Oral communications: Ability to comprehend and communicate complex information in English to multidisciplinary care team members, patients, caregivers, and outside providers, including presentations during weekly program meetings.
  • Team Work: Ability to create a collaborative and respectful team environment while constantly improving workflows and exhibiting skills of flexibility, organization, and communication skills in a small group.
  • Autonomy: Ability to be self-directed, work independently and be accountable for responsiveness to patient and program needs.
  • Must be able to function well in a busy environment, and to juggle multiple responsibilities.

  • WORKING CONDITIONS:

    Works in a variety of patient care environments where there may be exposure to communicable diseases and hazardous materials such as chemotherapeutic agents, radioisotopes, and radiation. Work entails walking and lifting. Required to Work Weekends Per Contract.

    SUPERVISORY RESPONSBILTIES: None.

    Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff
