1. Assists with patient assessment and care provision under the direction of the Registered Nurse
o Measures/records patient information (hgt, wgt, v/s)
o Specimen Management - Obtains, prepares and preserves a specimen for a laboratory test
o Observes/reports or records patient response to care given
o Identifies and reports deviation in v/s or patient observations & patient needs to RN
o Bedside lab testing - Performs laboratory tests at the bedside or POC.
2. Prepares treatment rooms for each patient.
o Feeding, provides nutritional intake for patient who is unable to feed self
o Passes, collects and checks food trays
o Assists patients with bedpans/urinals/commode
o Tube Care - Assists with management of a patient with an external device exiting the body
3. Patient Care
. Transports - moves a patient from one location to another
o Self-Care Assistance: Transfer - assists a person to change body location; assists with the use of assistive devices as indicated
o Exercise Therapy: Ambulation - promotes and assists with walking to maintain or restore autonomic and voluntary body functions during treatment and recovery from illness or injury
o Assists with dressing and undressing
4. Assists in promoting patient safety & environmental cleanliness/order
o Uses side rails appropriately
o Environmental Management - Manipulates the patient's surroundings for therapeutic benefit, sensory appeal, and psychological well-being
o Makes beds
o Cleans and cares for equipment
o Carries out transmission-based precautions
o Observes oxygen precautions
o Emergency Care - Provides life-saving measures in life-threatening situations
o Maintains known precautions- latex allergy, MRSA, seizure, etc.
o Maintains understanding and use of adaptic equipment per the ADA.
5. Essential Functions
o Patient Rights Protection - Protects health care rights of a patient, especially a minor, incapacitated or incompetent patient unable to make decisions. Protects patient privacy.
o Anxiety Reduction - Minimizes apprehension, dread, foreboding, or uneasiness related to an unidentified source of anticipated danger
o Visitation Facilitation - Promotes beneficial visits by family and friends
o Answers call bells promptly reporting patient's need as indicated
o Provides orientation to room/unit
o Assists patients with communications (I.e: nursing call system, phone, etc.)
o Provides diversional activities
o Provides comfort measures (I.e.: back rubs, music, reading material)
o Assists with newborn web pictures
o Assists with newborn care and transport
6. Acquires and maintains current knowledge and competency in LNA practice
o Acquires and maintains clinical knowledge and skills appropriate to the assigned area and practice setting
o Maintains personal documentation of in-service attendance
7. Communications: Uses available resources to communicate with members of the multidisciplinary care team.
o Greets patients, physicians, visitors and staff who arrive on unit.
o Uses transport call system to coordinate transport services.
o Calls stat orders to ancillary depts
o Transfers calls to appropriate parties.
o Answers phone by giving unit, name, and title.
o Upon discharge makes follow up appointments and arranges for tests and procedures.
o Uses Nurse locater/unit paging system appropriately
o Maintains patient census tool
o Maintains computer census (i.e. ADT functions).
8. Demonstrates competency in performance of procedures and skills under the direction of the RN
o Glucometer
o Pulse oximetry
o Automatic blood pressure device
o Simple dressing change
o IV Removal
o Sequential Compression Device Operation
o Foley removal
o Application of heat and cold
o Restraints
9. Physician Order Entry
o Enters physician orders via computer according to priority ( stat, routine, pre-formatted).
o Processes and schedules daily labs, diagnostic tests and procedures.
o Checks charts for new orders that need to be transcribed
o Signs off orders (e.g., signature, title, date & time)
o Follows computer down-time procedures.
o Processes patient charges for supplies and procedures.
10. Unit Operations: Utilizes clerical and organizational skills to assure smooth functioning of department operations.
o Maintains stock of patient care supplies and equipment.
o Maintains a supply of chart forms or is able to access / print computerized forms
o Maintains stock of clerical / desk supplies.
o Documents equipment repair / maintenance needs.
o Participates in response to emergency situations (I.e., pages codes, calls physicians, etc).
o Checks/refills paper supply, toner in printers and copiers.
o Sends and receives documents via fax machine.
o Duplicates documents using copy machine
o Orients new staff members to the unit.
11. Essential Functions
o Files forms and reports appropriately in patients chart.
o Restricts access to patient information (i.e. charts & computer)
o Accurately labels forms with patient identification.
o Assembles patient charts upon admission.
o Assembles necessary forms for patients being transferred to an external facility.
o Prepares patient charts for transfer to other units.
o Puts chart in appropriate order, and sends to medical records.
Qualifications
Experience Minimum Required
• 3 months clerical; 3 months as LNA
Experience Preferred/Desired
• Over one year clerical and transcription of orders; LNA experience in an acute care setting
Education Minimum Required
• High School graduate with office practice courses; graduate of an LNA program
Licensure and/or Certifications Required
• BLS, CPR, LNA
