Summary:
The Patient Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Patient Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care.
Responsibilities:
Collect and manage all relevant clinical data (intake forms, photographs, patient health history, labs, images, etc.) prior to consultations/surgeries or procedures. May assist with obtaining authorizations, scheduling appointments and coordinate care between providers, where appropriate, connecting patients to available services, such as primary care providers, specialists, and other community health resources.Assisted patients in coordinating care with other medical professionals. Coordinates patient care plans, including scheduling appointments, following up with providers, and providing/obtaining referrals; helps patients overcome obstacles that they may encounter while pursuing treatment. Coordinates a wide range of activities and services to support the patient by utilizing a variety of communication methods including but not limited to telehealth, face-to-face, text, telephone, or e-mail.Provides educational resources to patients on their medical condition, treatment plans and other health-related topics as directed by the clinical team and is a resource for the patient on the insurance and administrative aspects of their peri-operative/peri-procedural care.Works closely with physicians and the multidisciplinary care management teams. Facilitates care coordination, and documents efforts. Extract data from medical record sources to gather information and perform outreach. Communicates details to the appropriate team, providers, and/or supervisors.

Complexity of Work: Success will be measured by the results of the process and outcome performance measures of the population of patients. Requires critical thinking skills, effective communications skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Knowledgeable in Excel, Word and PowerPoint and able to use electronic medical records such as EPIC. Required Work Experience: One (1) year of experience in a healthcare setting required.
