Job Summary:
Ensures accurate and complete coding of diagnoses that fall into the Hierarchical Condition Category (HCC) model, as necessary for Medicare Advantage plans and other risk-adjusted (RAF) guidelines, through clinical assessment of the patient chart & preparation of that chart for the provider, active education, provider training/communication, and monitoring for effectiveness. Applies all departmental coding quality guidelines and meets productivity standards: reviews and performs HCC coding at an average of three medical charts per hour, while maintaining a 95% overall accuracy rate. Appropriately uses HCC principles set forth by Medicare Advantage plans to evaluate use of most specific and accurate codes and understands that the organization's strategic initiatives (financial, statistical, and quality benchmarking) rely on the accuracy and integrity of all data reviewed by them. Other duties as assigned.
EXPERIENCE DESCRIPTION:
Recent experience in HCC coding is required. Experience in Medical Surgical or primary care nursing with emphasis on chronic conditions. A minimum of 3 years of HCC coding experience at an academic facility is preferred. Experience in care management, care coordination, wellness visits is preferred.
EDUCATION DESCRIPTION:
Associates degree in Nursing is required. Associates Degree in HIM, Coding Program Certificate Completion or other evidence of HCC coding training is preferred. CCS-P (Certified Coding Specialist Physician), CPC (Certified Professional Coder), or CRC (Certified Risk Adjustment Coder) is preferred.
SPECIAL SKILLS DESCRIPTION:
Professional knowledge of nursing theory and practice. The ability to adapt to and use tools to support coding functions including the encoder, EPIC EMR, and other software applications is necessary. Effective communication, written and verbal, is vital to this position. Ability to adapt to change, shift between tasks without losing productivity, and ability to work as a team is extremely important to this role. Creative solution-based problem solving, critical thinking skills and clinical judgement skills are essential to this role. ICD-10 Diagnosis Coding, Medical Terminology, Anatomy & Physiology, Pathophysiology, Pharmacology, HIPAA knowledge required.
LICENSURE DESCRIPTION:
Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
