Position Summary:
-We are seeking an experienced Medical Case Manager (LTSS) to join our team on a 6-month contract basis. This role focuses on case management, care coordination, and utilization management for members in Long Term Support Services programs, including Community-Based Adult Services (CBAS), CalAIM, and those in long-term care facilities. You'll play a vital role in communication and coordination among care teams to ensure quality and cost-effective outcomes.

Key Responsibilities:
-Medical Review & Support: Conduct assessments, authorize services, and coordinate care using tools like PASRR, MDS, and HRA.
-Collaborate with partners to implement community support services.
-Administrative Support: Identify staff training needs and maintain data resources.

Position Details:
-Schedule: Full-time, in-office, Monday-Friday, 8:00 AM - 5:00 PM
-Location: Orange, CA
-Pay Rate: $60-$70/hour

Qualifications:
-Required: Active California RN license.
