Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The SNAP Outreach Coordinator will provide support to the Supplemental Nutrition Assistance Program (SNAP). Primary focus will be outreach to patients and communities within the Eskenazi Health Clinic neighborhoods. This position will also work in partnership with the Eskenazi Health Fresh for You Market Well on Wheels by supporting customers that have questions pertaining to SNAP enrollment and eligibility.

Essential Functions and Responsibilities

  • Proactively contributes to Eskenazi Health mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County. Models Eskenazi Health values
  • Conducts face-to-face SNAP outreach, and application assistance to patients and community members at select locations, as well as over the phone. Follows procedures necessary for maintaining strict neighbor confidentiality.
  • Provides detailed explanation of SNAP eligibility and benefits to patients, community members and staff.
  • Ability to utilize language translation services to assist patients and community members that speak English as a second language.
  • Disseminates informational materials developed for and provided by Eskenazi Health for SNAP outreach marketing purposes. Assists with educating community partners about SNAP and its importance to patients and community members.
  • Works with the Eskenazi Health Social Determinants of Health team to identify high-volume locations for conducting SNAP outreach.
  • Performs neighbor follow-up as needed for successful application completion, including but not limited to preparing, mailing and submitting application packets to the Indiana Family and Social Services Administration (FSSA).
  • Makes referrals to outside organizations for neighbors based on individual needs (i.e. food, utility assistance, public assistance, housing, education/training, employment, mental and physical health, transportation, child care etc.)
  • Assists in helping to track and report on SNAP-related metrics and key program deliverables.
  • Provide timely response to agencies at all times and satisfactory resolution of any issues.

Job Requirements

  • Minimum of high school diploma or GED required
  • Minimum of three years' work experience in a related field
  • Current Indiana driver's license, proof of automobile insurance, and a dependable car
  • Willingness to work with patients and their family members or other support persons in the health center, home settings, and community settings
  • Experience in customer service or quality assurance.
  • Experience with MS Excel/Word/Survey Monkey/Outlook and Google documents.

  • Must be able to relate to a variety of people with different cultures, socioeconomic backgrounds and education levels
  • Ability to establish and maintain effective working relationships with superiors, coworkers, patients and families
  • Basic Computer Skills
  • Ability to work independently
  • Knowledge of available community resources and required application processes
  • Ability to make sound decisions regarding need and urgency of referrals
  • Ability to use age-appropriate communication skills
  • Must be able to work effectively in a sometimes stressful and unstructured environment
  • Ability to maintain accurate customer files and complete required reports. •
  • Bilingual preferred, but not required
