CLINICAL SOCIAL WORKER / PART TIME 20 HOURS/ CHARLESTOWN

About this CLINICAL TRIAGE AND REFERRAL COORDINATOR job:
  • Provides 20 hours per week of clinical administrative services to the Counseling and Behavioral Health unit at the MGH Charlestown Health Center.
  • The position provides triage and assessment for all new referrals to the Behavioral Health unit. All new referrals are contacted, basic needs assessed, risk assessed, referrals and additional information provided as needed.
  • This role works closely with administrative staff to manage departmental waitlists and assign new patients to clinical staff members. The role also supports case assignment during care transitions within the Health Center and interacts with referring providers as needed to adequately address patient needs.
  • Management of EHR referral functionality and collaboration with administrative staff is required.
  • Opportunities to participate in clinical education of social work students and on Departmental, Health Center, and Hospital committees are available. Considers the appropriate and efficient use of Health Center resources and supports implementation of innovations in care delivery as appropriate
PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Practice
  • Provides brief assessments of patients and families at the point of referral, and ascertains patient needs and wishes at that time. Is able to make rapid assessment of patient safety risk and with support of clinical staff, execute safety protocols as required.
  • Provides care that is evidence based, incorporating updated clinical care models.
  • Provides care that is culturally/racially sensitive, and trauma informed. Advocates on behalf of patients and/or their families to gain access to services and resources.
  • Maintains comprehensive referral resource information to provide to those who request referral assistance.
  • Assists in case assignment and care transitions as needed. Able to navigate referral processes within electronic medical record and work closely with administrative staff to facilitate referral.
  • Presents and/or discusses clinical work in formal and informal settings and seminars. Seeks clinical consultation as needed.
  • Provides risk management. Facilitates the appropriate and efficient use of Health Center and Hospital resources.
  • Takes Urgent response duty as assigned; performs other duties as assigned. May participate in research projects.
Collaboration
  • Collaborates and consults with patient, families, professionals, and providers inside and outside the Health Center to develop and implement outpatient care. Collaborates with community-based resources as needed.
  • Participates as a key member of integrated medical and behavioral health care teams, participating in treatment planning and providing outpatient therapy.
  • Refers to and collaborates with interdepartmental specialists and programs, outside agencies, facilities, payors, and other relevant community resources.
Organizational/Administrative Skills
  • Takes responsibility for own administrative duties, including timely and appropriate documentation in patient medical records. Monitors and manages productivity to established standards.
  • Attends and participates in unit meetings, division meetings, and staff meetings.
Professional Conduct
  • Adheres to the profession's clinical standards and code of ethics. Adheres to the Department, Health Center and Hospital standards, policies and procedures.
  • Maintains the standard and educational requirements specific to the position and professional licensure.
  • Promotes Departmental, Health Center and Hospital public relations.
Professional Development
  • Participates in supervision as appropriate to his/her level of professional development. May assist with or supervise undergraduate and graduate social work students and staff.
  • Is responsible for own professional development. Identifies own learning needs and goals. Attends pertinent continuing education and professional activities related to job responsibilities and career goals. Keeps current with social work and health care developments pertinent to job and profession.
  • Responsible for own professional growth and maintenance of required credentials.
  • Takes responsibilities for leadership and participation on committees and task forces of the DON and the hospital.
  • Maintains and updates clinical knowledge and skills based on current health care practice/research.


Qualifications
QUALIFICATIONS
  • Master's of Social Work (MSW) from an accredited program required.
  • Massachusetts Licensed Clinical Social Worker (LCSW) OR Licensed Independent Clinical Social Worker (LICSW) required.
  • Bilingual in Spanish preferred but not required.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED
  • Strong assessment and treatment skills, utilizing evidence-based approaches and protocols.
  • Excellent organizational skills.
  • Strong time management skills; ability to re-prioritize tasks as needed to meet demands with little supervision.
  • Demonstrated ability to communicate effectively orally and in writing.
  • Demonstrated ability to navigate electronic health records.
  • Strong interpersonal skills enabling effective team collaboration.
  • Demonstrated ability to understand the role of social worker in a community healthcare setting and to work in a complex, fast paced environment.
WORKING CONDITIONS:
  • Able to provide assessment services in office or remotely using virtual technology.
  • Able to communicate orally and in writing.
  • Able to document services provided using electronic medical record technology.
SUPERVISORY RESPONSIBILITY No supervisory responsibilities at start.
